Please fill out all information in its entirety. Easiest to do from a laptop or desktop computer. In order to book an appointment, we have to have all contact information & vaccinations entered. To prepare, have the following ready:
🐾 Vaccinations records for all of your dogs (Rabies) We’ll gather any other vaccinations during your appointment
🐾 Veterinary Information (name, address, phone)
🐾 A photo of your pet that you can upload
🐾 Health & service flags boxes checked, allergies noted, and additional notes filled out to help us learn more about your pet. (This is all in the application)
🐾 Please check your email for a verification email. By verifying your email, we can send you email reminders about upcoming and future grooming appointments. Missed the email? Check your junk mail or Contact: verify@pawfinity.com When you receive your text or email about your appointment, please open it, click on the link and confirm your appointment.
You can't go back and enter vaccinations later. So please upload Rabies and we can enter other vaccinations on our end when we arrive for your appointment. Or you can text a picture of your dogs latest vaccinations to 612-470-6006. Please inform me who they are for with your first and last name. This form should take 15 min or less to fill out
