CLIENT GROOMING AGREEMENT
You will be asked to read and sign Puppers, LLC’s (hereinafter “Puppers”) Policy Agreement prior to any grooming services (the “Services”) being performed.
Payment is due when Services are rendered. Puppers accepts cash, checks and all major credit cards. Checks returned NSF WILL incur a fee of $35, plus any bank fee charged to Puppers’ account. Puppers reserves the right to refuse to provide further Services until such fees are paid.
For established monthly clients (clients obtaining services of at least 2 times per month for a minimum of four (4) months, Puppers may agree to bill on a monthly basis. For clients on monthly billing, payment is due by the 10th of each month. Puppers reserves the right, upon thirty days’ notice, to cancel monthly billing and require payment at the time Services are rendered.
CANCELLATION POLICY/NO SHOW/TRIP FEE
It is Your responsibility to keep track of scheduled grooming appointments. Puppers will send a courtesy text or email 2 days prior to Your appointment to confirm and remind You of the scheduled time. If you fail on Your first grooming appointment fail to have Your pet available when groomer arrives for Your scheduled appointment, Puppers may elect not schedule any future appointments.
If you cannot keep your grooming appointment, a minimum of 24 hours advance notice is required. Failure to call and provide 24 hours advance notice before your scheduled appointment or a failure to have Your pet available at the scheduled time WILL result in a cancelled appointment charge of $50, which must be paid within 7 business days.
In the event that multiple pet are scheduled for a grooming appointment, and one or more pets are not groomed at Your request, then You will be responsible for 50 % of the fee for the scheduled Services for each pet not groomed.
Grooming appointments are scheduled for a 1-2-hour arrival window. As a mobile service, our schedule is subject to interruptions and delays, such as, but not limited to: refueling, traffic, driving distance, uncalculated delays. If the groomer anticipates being more than 15 minutes early or late, Puppers will call to provide an update on the anticipated arrival time.
LOCK BOX/LATCH KEY SERVICE
As an accommodate to Puppers’ clients, Puppers offers a “Lock Box/Latch Key Service”. At Your request, Puppers will provide a lock box or You can provide a lock box with a combination. Puppers’ charges a fee of $5 per visit for the rental of its lock box. If a lock box is to be used You must provide a key, a garage code, or some other means of access to the home. When a lock box is used, the groomer will arrive at your home at the scheduled time.
If You are not set up for monthly billing, You will need payment at an agreed upon location.
When utilizing a lock box, Your pet must be crated or left in an accessible area.
As Puppers is utilizing the lock box as an accommodation to You, You agree to hold Puppers harmless for any claims of theft, loss or damages while the Puppers’ groomer is providing Services in your home for Your pet(s).
All dogs must have current Rabies vaccination from a veterinarian on file with Puppers. Puppers recommends that dogs should also be vaccinated for Distemper/Parvo and Bordetella. Puppers, LLC will not provide Services for pets that have not be vaccinated for Rabies and shall not be responsible for pets that have not been vaccinated for Distemper, Parvo and Bordetella.
AGGRESSIVE OR DANGEROUS PETS:
Client must inform Puppers, LLC prior to grooming if your pet has bitten someone or has aggressive tendencies. Puppers, LLC will not accept any aggressive dog and reserves the right to discontinue providing Services during the grooming appointment and/or in the future. If You provide false information regarding pets’ aggressive behavior or the pet shows aggressive behavior during the grooming, we reserve the right to discontinue services and You will be responsible for the full grooming charge plus a $25 Aggressive Dog fee.
You will be liable for any bites or any property damage caused by their pet(s) as provided by law.
For the safety or Your pet and our groomers, Puppers will not be able to provide Services for OVERLY AGGRESSIVE OR OVERLY STRESSED PETS, PUPPERS, LLC WILL NOT BE ABLE TO MAINTAIN YOU AS A CLIENT.
It is Your responsibility to inform Puppers of Your pet’s pre-existing conditions (including but not limited to advanced age, extreme nervousness, neurosis, illness, previous injury, skin or coat conditions, or medical conditions).
. Occasionally, grooming can expose a hidden medical problem or aggravate an existing condition. This can occur during or after grooming and may require immediate medical attention. In the event that Puppers is unable to contact You to regarding Your pet’s health or condition that has been aggravated by the grooming, if Puppers deems it to be in the best interest of your pet to seek immediate medical treatment, You designate Puppers as Your agent to obtain services of a veterinarian, the cost of which shall by Your expense.
Grooming procedures sometimes can be stressful, especially for a senior pet or a pet with health problems. Because senior pets and pets with health problems have a greater chance of injury, these pets will be groomed for cleanliness and comfort, in styles that will not add to their stress. Puppers will not be responsible for accident or injury to an elderly or health-compromised pet during their grooming.
Puppers will make a reasonable effort to removed mats from the coat of your pet. If it is deemed unsafe and unduly stressful for the pet, after consultation with You, the decision may be made to shave the coat remove the mats*. If You do not agree with having the coat shaved, the balance of the grooming will be performed and the full grooming fee will be charged.
If it is determined that mats can be removed safely with little harm or stress to the dog, a de-matting fee of $45 per hour will be charged as well as the full grooming fee.
*The removal of a dog’s coat due to heavy matting has the following risks: mild to moderate skin lesions from clipper blades, abrasions of warts, moles or skin tags, skin irritation, redness, itchiness, excessive licking, alopecia. Shaved pets are also prone to sunburn.
FLEAS AND TICKS
You are responsible for keeping Your pet(s) flea and tick free. If a flea or tick is found on your pet, Puppers will administer a Nitenpyram (Capstar, FastCap, CapGuard) pill at a charge of $40. This is to kill any fleas on your pet as well as to maintain the sanitation of the mobile dog spa. If more than 10 fleas or ticks are found on Your pet, a sanitation fee of $100 (includes the cost of the Nitenpyram pill) will be charged.
(Possible Side Effects of Nitenpyram include: vomiting, depression/lethargy, pruritus (itchy, scratchy skin), urticaria (wheals, hives), diarrhea, anorexia (loss of appetite), and skin congestion (red skin).)
You consent to Puppers taking pictures of your pet, before and after grooming, and utilize the same for its website, social media and/or other promotional or advertising purposes as determined by Puppers. Client affirms they are the rightful legal owner, or caregiver to the pet for which services are rendered.
I have read, understand and agree to the above terms for the grooming and maintenance of my pet(s). In consideration of the Services to be provided, I agree to hold Puppers harmless from any and all damages, losses, or claims or injury to my pet, and/or my personal or real property, including death, injury or shock to my pet. I acknowledge that the interpretation of this provision is to be read in the broadest since possible and encompasses any real or proceeded negligence and all acts performed reasonable within the scope of delivering the Services by Puppers, its agents, its employees or representatives. The terms, special services or handling shall include but are not limited to emergency veterinarian services in the event I am not available.
I authorize Puppers and/or its agent thereof to act as my agent in the event emergency veterinarian services, boarding, care-taking, and/or transportation as is necessary and I agree to pay all costs upon demand by the service provider.
