Shot Records

Current shot records are required for Grooming and Boarding. Please be sure to email a copy of your pet's current vaccinations to petc-mgr@earthlink.net or bring them in when you drop off. Your pet(s) will need to be current with Rabies, Distemper, Parvo and Bordetella in order to be groomed and/or boarded. If you have any questions regarding their vaccinations do not hesitate to call. We will always help answer any questions you have.

To Insure You Receive an Appointment Confirmation

When filling out the form below, under the Contact By: section please remove the check from SMS/Text (leaving the email and phone choices both checked) and select Email under the Preferred Contact section. Since we do not yet use text notifications, doing this will insure your appointment is confirmed. You will not receive any junk email from us. This is strictly for confirmation purposes.
