Mobile Grooming Liability Waiver
Your animal is very important to us. Jim’s Mobile Dog Grooming (“JMDG“) would like to assure you that every effort will be made to make your animal's grooming experience as safe and pleasant as possible. Safety comes first for everyone, people as well as the animals, during the grooming process.
Health or Medical Problems
Occasionally, grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming. All medical expenses for veterinary care will be covered by the animal's owner upon signing this contract/agreement.
Although accidents are very rare, there is a risk when dealing with pets. Your pet’s safety and comfort are our number one priority. In the event an accident does occur, you will be notified of the accident. If JMDG feels it is serious, and the owner is not on-site, JMDG will seek immediate veterinary care for your animal. Veterinary costs will be the responsibility of the animal's owner. Client further understands and agrees to indemnify and hold harmless JMDG, its owner, employees and affiliates from and against any and all liabilities, expenses, damages, and costs, including attorney fees, resulting from any service provided or injury, including death to client’s pet(s), whether in our care or after our services have been completed.
Veterinarian Authorization-Medical Emergencies
This release gives JMDG full authorization to seek medical treatment from authorized veterinarians or nearest veterinarian in the case of any medical emergencies while in the care of JMDG. All veterinarian costs and expenses will be the responsibility of the animal's owner.
Animals will be current on their Rabies vaccination prior to a grooming session with JMDG. New clients will be required to provide proof of current Rabies vaccination, along with their veterinarian’s contact information, prior to services being rendered.
JMDG requires flea/tick treatment from April – November to enter the mobile grooming vehicle. In the event we find parasites such as, fleas, ticks and lice, JMDG reserves the right to refuse service to your animal.
Dangerous or Aggressive Animals-Refusal of Services
JMDG has the right to refuse any services at any time. In the event that your animal is too stressed or becomes dangerous to groom, JMDG has the right to refuse grooming services, stop grooming services, or cancel grooming services at any time before, during, or after grooming and client will be charged the full grooming fee. Failure to disclose that your animal has a history of biting, or is dangerous, will result in a fee of $250. Animal's owner will be responsible for all payments of medical expenses, emergency medical treatments, hospital costs, and recovery costs including but not limited to physical therapy, incurred by JMDG or any person employed by JMDG for any harm or damage caused by owner’s animal. JMDG does not handle dogs with a history of biting or aggressive behavior.
Use of Muzzles
JMDG will advise if muzzling is necessary. Muzzling does not harm your animal and protects both the animal and the groomer. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue. If an animal still acts in a way that is dangerous, JMDG has the right to stop grooming services at any time and full-service fee will be collected.
Interruptions During Grooming Services
For the safety of the animals being groomed, as well as the professional animal groomer, it is asked that clients do not interrupt the professional animal groomer during grooming. Please do not knock on the door or enter the grooming vehicle while your pet is being groomed, and please do not talk to your pet or allow him/her to see you during the grooming process. Every effort will be made to insure all pets are groomed as safely as possible, but an excited or distracted pet can be dangerous to continue to work on. If client insists on talking to their pet or the groomer during the grooming process, we reserve the right to end the grooming session, even if the groom is not completed and the full grooming price will be charged.
Grooming While Animal's Owner/Caretaker is Away From Their Home or Property
This release authorizes JMDG to perform scheduled grooming appointments while you are away from your home or property. In the case where JMDG is given a house key to your home, JMDG will not be held responsible for any damages to your home or property.
Home or Property Damages
JMDG will not be held responsible for damages occurring to plumbing, electrical, flooring, or any other part of client's house, home, or property.
Cancellations without providing a minimum of 48-business hours’ notice will incur a fee of $50. Business hours defined as Monday-Friday 9am-5pm, not including holidays. If the groomer arrives for a scheduled, confirmed grooming appointment and there is no one home to release your pet, there will be a $50 fee for travel time, gas & inconvenience. If you are unable to be home during your appointment time, we would be more than happy to make arrangements to groom your dog when you are away.
It is very rare for us to cancel appointments but occasionally we might have a mechanical failure with the vehicle preventing us from providing services. Cancellations can occur at any time by JMDG due to weather, road conditions, equipment failure, etc. In the rare instance that this happens, clients will be notified via email.
This release authorizes JMDG to take photos of your pet for client file and for advertising purposes. All photos taken are the property of JMDG.
Your satisfaction is important to us. If you are unhappy for any reason, and would like something adjusted, our groomer will be happy to make any adjustments while at the appointment. Once, however, the groomer leaves the appointment, any requests to return will be treated as a new appointment and client will be charged the full grooming fee. There are no refunds for grooming services. All sales are final.
I have reviewed this Service Contract for accuracy and understand the contents of this contract. I affirm that I am the rightful legal owner of the dog(s) for which services are being rendered. I authorize this signed contract to be valid approval for future grooming services, permitting JMDG to accept telephone reservations or emails for service without additional signed contracts or written authorization. I understand that pricing is subject to change. I have read, signed, and agreed to the aforementioned Policies and Procedures that are a part of this agreement.
