*ATTENTION* Even though this application shows that listing Vaccination History as
"optional" we do require your pet have a current rabies vaccine. Please list the rabies # and exp. date as well as providing your current veterinarian information.
** ALL Appointments require a $50 trip charge. This charge will be fully credited toward your total balance for pet's groom. Bookings will only be confirmed once the trip fee is paid. If you must cancel your pet's appointment we ask you give us a full 48 hour notice; at that time you may reschedule and your $50 will transfer to that booking. If 48 hour notice is not given, you forfeit our trip charge.
***If we come to your confirmed appointment and no one is at home, (Remember our groomers will make every effort to be on time; however, due to unforeseen occurrences when working with live animals and the possibility of traffic hold ups, weather delays, fuelstops,, etc., Heaven Scent reserves the right to have a 2 hour window from your (actual) scheduled time (see example below). If you are a "no show" your card will be billed for the entire amount of the confirmed groom. If your groomer feels his/her delay will take longer than the 2 hour window, we will call and either reschedule your groom appointment or refund you, your choice.
EXAMPLE OF THE 2HR. WINDOW: Appt. 12:00pm - Groomer is not late until 2:00pm.
