Names and phone numbers are stored in a password protected mobile phone which is held and only accessible by approved employees of Doggy Lodge Grooming and is only used for the use of contacting you in relation to appointments.

Information/release form for Doggy Lodge Grooming
At Doggy Lodge Grooming we groom humanely and we abide by the Animal Welfare Act 2007.
Sometimes it is necessary for a severely matted coat to be shaved off completely, if the matts are tight and close to the skin it would be extremely distressing for the dog to be “Dematted” and ultimately at Doggy Lodge Grooming your dog’s wellbeing is always paramount. Many dogs are traumatised by unnecessary hours of dematting, putting them off grooming for life. Shaving your dog is by far the kindest thing to do if the coat is heavily matted. If we find your dog to be matted once we have started grooming we will contact you to inform you that we intend to shave your dog. If we cannot get hold of you we will shave your dog in the interest of welfare. Please see Client consent form.
In this case we aim to work together with you and your dog as the coat regrows (and they do very quickly) to ensure we maintain a healthy new, matt free coat.
A Matted Coat can cause many nasty infections and infestations for example:
Hotspots & sores
Fungal & bacterial infections
Mites & lice
Urine & Faeces scalding
Eye & ear infections
Once the coat is removed, the skin can be red due to the lack of air circulation and dirt that has accumulated underneath. Occasionally there are sores or infections, this may need veterinary treatment. Dogs tend to scratch and this may lead to broken skin. If in doubt, speak to your vet.
Please note our normal charge does not include de-matting dogs, this may be charged for an extra cost.
We are more than happy to offer advice on keeping your dog’s coat matt free. It may be difficult to maintain a dog’s coat, particularly low shedding coats, like the very popular poodle crosses. However with grooming at home and regular visits to the salon it can be done. It is essential for the wellbeing of your dog.

Teeth cleaning consent form
I understand that the cleany teeth ultrasound toothbrush will be used on my dog which has no vibration, no movement and no noise.
At the start an assessment will be done of your dogs teeth to check your dogs teeth are suitable for the toothbrush to be used on.
The session may be stopped if :
Any loose teeth are suspected
If the dog objects and becomes distressed or aggressive
Any obvious abnormalities are visible or suspected in the mouth such as a foreign body embedded in the mouth, oral warts, oral injuries or tumours
Full payment for that session will still be paid if the session is stopped
I understand that:
Their may be some minor bleeding of the gums during or after teeth cleaning as the tartar and plaque is removed
A rubber descale will be used in a kind and gentle manner to remove loose tartar, plaque or staining from the teeth.
I Confirm that:
I do not know of any medical reasons why teeth cleaning can not be carried out on my dogs teeth and I acknowledge that it is my responsibility to inform staff of Doggy Lodge Grooming in future should this be the case.
I am aware that ultrasound teeth cleaning is not a substitute for veterinary inspection and Doggy Lodge Grooming staff are not qualified to diagnose abnormalities or disease.
I consent to the above and confirm I am the legal owner/guardian of this dog
Terms and conditions
Welcome to Doggy Lodge Grooming where your dog’s welfare is my number one priority. Under the animal welfare act 2006 we are obliged by law to follow their guidelines and implement a very comprehensive record keeping policy system. This is to protect the welfare of the dog, but also protects you as our client and us as your service provider.
Please fill out and sign the consent form below at your dog’s first session. We will then keep it in your dog’s individual file, along with any other relevant information.
If your dog’s coat is badly matted we will assess if the Matts can be removed by grooming. There will be a charge of £10 for the extra time this takes. If we feel the coat is too badly matted to groom without causing undue discomfort to the dog, we reserve the right to clip the dogs coat using clippers. This will result in a very ‘close’ cut as the bad Matts need to be removed close to the skin.
Aggressive/anxious dogs/staff safety
We reserve the right to refuse to groom a dog that becomes aggressive during the grooming process. If a dog shows signs of aggression and we are concerned for either our own safety or that of the dog. We will terminate the session immediately and contact you. We may try the groom with either the aid of a cone or muzzle however should we still not be able to continue will contact you to collect your dog and the full price for the groom will still be payable. Please inform us if your dog has bitten or snapped or is overly anxious. We can then decide on the best way forward for your dog causing the least stress for everyone.
It happens from time to time that you may not notice your dog has fleas. If we find this to be the case your dog will be treated immediately with a flea treatment shampoo. This will incur an additional £10 to the price of the groom.
Duty to disclose
You have disclosed and will continue to disclose on an ongoing basis, any and all medical or other conditions, including but not limited to personality concerns or behaviours that may affect, limit or prevent your dogs from being groomed.
Doggy Lodge Grooming employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects of the grooming process, including but not limited to: clipper burn, exposure of the quick, reactions to flea treatments and de-shedding dip treatments, hair splinters, and sharp nails (after clipping). By signing this agreement, you (the client) agree to relieve Doggy Lodge Grooming, and it's agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by Doggy Lodge Grooming. The pursuit of any veterinary, behavioral, or rehabilitational services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve Doggy Lodge Grooming and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by Doggy Lodge Grooming and it's agents.
Anal Glands/Ear hair plucking
Doggy Lodge Grooming does not provide anal gland expression or ear hair plucking as part of any service offered. It is our philosophy that this should only be carried out by a licenced veterinary person, and only when medically necessary.
Your dog maybe photographed before, during and after a groom. This may be used on our website, social media or other marketing material. Should you not wish for this to happen please let us know.
We use 100% pure aromatherapy oils which are safe for dogs but please let us know if your dog is epileptic, seizure prone or pregnant. Or if you feel your dogs will not react well to it.
Please be punctual. We work to appointment only. We also offer a free text reminder service. Most grooms take between 1-3 hours or more depending on the size and temperament of the dog and the condition of the coat. You will be advised or phoned what time to collect your dog.
We request 48 hour’s notice if you need to cancel your appointment. We may charge up to 50% of the groom if the appointment is within 24hours notice. Should this happen more than once a deposit may be required for future bookings
Payment will be made on collection of your dog. We currently only take cash payments. Estimates are based on the average dog of the breed but the owner must remember that some dogs may require extensive extra work and that will charged for on an hourly basis and must be collected at the time stated.
If you are dissatisfied with any part of your dog’s groom please let us know within 3 days of the groom (preferably before you leave). Reasonable attempts to alter the trim will be dealt with immediately or changes noted for the next groom. Please don’t leave negative feedback without contacting us first.
