Deogee Client Agreement
Veterinarian Release and Billing Agreement: I give Deogee the right to transport and allow a veterinarian to render services to my dog if Deogee deems my pet needs immediate veterinarian attention. Furthermore, I agree to pay all vet bills in full, in addition to, any grooming charges incurred.
Behavioral Problems and Aggressive Dogs: Dogs are often distracted, unpredictable and can show anxiety when in an unfamiliar situation. Sometimes this unpredictable behavior can result in accidental injuries. I understand that for everyone’s safety, my dog may need to be muzzled. I understand that muzzling will occur at the groomers’ discretion. If at any time the groomer feels that the dog is a safety threat, grooming will cease, regardless of the degree of completion. The pet owner is responsible for the entire cost of the groom, regardless of the degree of completion. I, also, understand that if my dog injures another pet or person I am fully responsible for medical costs incurred.
Overweight, Elderly and/or Handicapped Dogs: The groomer cannot effectively groom if they are required to support the body-weight of a dog, and the finished product of the groom (completion or look) may be compromised. We are willing and able to make concessions for dogs who cannot tolerate the physical stress of grooming (i.e. allowing the dog to sit or lay for portions of the groom, taking frequent breaks), but we ask for your patience in your pet's length of stay in our facility.
Shaving/Cutting Hair Short: Shaving a dog requires using a blade that cuts close to the skin, and sometimes this can cause minor skin irritation, abrasions, hot spots and razor burn. We are happy to shave your dog but will not be held responsible for any medical issues caused by shaving. We do not recommend shaving double coated dogs for any reason. Shaving a double coating dogs including, but not limited to, Golden Retrievers, Shepherds, and Labs, can result in long term and irreversible damage to the coat. The coat may grow back patchy, coarse, or not at all. We will shave any dog at the customer’s request. However, we are not responsible for long term damage or skin irritations to the coat of a dog that should not be shaved.
Anal Glands: As groomers, we can only express glands externally; this can cause irritation and rupturing of anal glands. We do not automatically express the anal glands but will do so at the owner's request.
Severely Matted Dogs: If requested, a “de-matting” fee will be added to my bill. Mats are tight knots in a dog’s coat that are often impossible to remove without cutting through or shaving affected areas. Matting can be extremely painful as it restricts blood flow, causes skin irritations and is extremely uncomfortable for the dog. The groomer has full discretion in determining whether the dog’s coat needs to be shaved. We will do our best to indicate to the pet owner, at time of drop off, whether shaving is necessary, however, often times we discover additional mats during the groom. We will not de-mat severely matted dogs. Depending on the severity of the matting, dogs can suffer nicks, cuts, and clipper burn from the removal of mats. Additional skin conditions may be revealed when mats are removed, including, but not limited to, hot spots and infections.
Dog with Infestations: After dropping off your pet for grooming services, we will inspect your dog’s coat and skin for the presence of fleas. If your dog is found to have fleas, we will take precautions to protect the other dogs on the premises, as well as our facility. You (the pet owner) will be required to pay for a flea treatment service (minimum of $15) and payment is due at time of pickup. The treatment is not a preventative; it only kills the existing fleas & their eggs.
Canine Influenza Virus Waiver: CIV (Canine Influenza Virus) & Bordetella (Kennel Cough) is often spreading in our community. By signing this waiver you understand and accept full responsibility for the risks associated with your dog being in a building with other dogs. You, also, fully understand and accept all responsibility for the cost of treatment if your pet contracts CIV. You, also, understand CIV vaccines do not fully protect your pet from the virus. If your pet is suspected to have CIV, we require immediate pick up of your pet, since this virus spreads extremely fast.
Daycare Policy: Upon drop-off of your dog for grooming services, we will give you an estimated time that your dog will be completed. We will then call you when your dog is finished. In order to keep the amount of dogs in the shop manageable, we ask that you pick up your dog within an hour of your phone call. If you do not pick up within the hour, or require that your dog stay longer, a Daycare charge of $25 per dog will be added as one of your services of the day. Daycare will include “Potty Breaks”, Water and Socializing with other dogs on a case-by-case basis.
“Potty Breaks”: In order to keep a sanitary environment, any dog that is at Deogee for an extended period of time, or a dog that we feel needs to relieve themselves, will be taken outside for a needed “Potty Break”. While Deogee puts every dog's safety first, we are not responsible for any unexpected mishaps outside our doors.
Cancellation / No Show Policy / Late: Appointments are taken throughout the day and are for the specific time you have reserved. If you are more than 15 minutes late, we reserve the right to call in another customer and possibly reschedule you for another day. If you do not show up for your appointment, or do not call to cancel within 12 hours of your appointment, you will be asked to pay an additional $15 “no show” fee per dog. The fee will be added to your next appointment, in addition to normal grooming costs. If you are a habitual “no show” offender, you may be asked to prepay for your grooming appointments. Your prepayments for any future appointments are non-refundable.
Returned Check Policy: Checks that are returned non-payable are subject to a $30.00 Service Fee & then future appointments must be paid in Cash PRIOR to the start of your pet's next grooming.
Social Media Release: By becoming a client you give permission to let your pet's photo be used by Deogee on social media platforms; such as Deogee's website, Facebook page, Instagram, etc. For privacy purposes, only the first name of your dog(s) will be mentioned. You have the choice to opt out of allowing your pet's photo to be used by a written request.
By completing and signing this page, I hereby release Deogee, and all associated parties, from any and all liabilities for injuries to myself, my dog, or any other property of mine, which may arise from any services or products provided by Deogee. I, also, acknowledge that I understand Deogee's Salon Policy.
Note: This form will automatically apply to any and all additional pets acquired by the above Pet Owner.
