CUTIE PAWTOOTIE DOG GROOMING LLC GROOMING RELEASE FORM
You and your pet are important to us. Because we care, we want to assure you that every effort will be taken to ensure your pet's visit with Cutie Pawtootie Dog Grooming LLC is as pleasant as possible. It is important to understand that many pets respond to grooming differently, even with the best efforts. We will make every effort to create a positive experience. Please read and acknowledge the following:
1: Your pet's safety and comfort is our number one priority. We do not muzzle unless your pet gives us a reason to. Muzzling does not harm your pet and protects both the pet and the GROOMER. In some cases, muzzling may even calm a stressed animal, allowing the grooming process to continue. Although accidents are rare, there is a risk when dealing with animals.
2: If your pet becomes too stressed or becomes dangerous to groom, the GROOMER has the right to refuse or stop the grooming services. In that case, the client will be charged a grooming fee for the services up to that point.
3: Accidents can happen. Grooming equipment is sharp, and animals can be active. Although we use extreme caution and care in all situations, possible issues could occur including cuts, nicks, scratches, quicking of nails, etc. In the event that an accident does occur, you will be notified.
4: Grooming can expose a hidden medical problem or aggravate a current on. This can occur during or after grooming. SENIOR PETS have a greater chance of injury during grooming.
5: If the GROOMER feels that an accident is serious or that a dog needs medical attention, and the owner is not on-site, we will seek immediate veterinary treatment, at your expense, should it become necessary. All medical expenses for veterinary care will be covered by the pet's owner upon signing this release aggreement.
6: Pets with MATTED or TANGLED FUR have a GREATER RISK OF INJURY, STRESS, and TRAUMA due to DAMAGE that MATTING causes. In some cases, matted fur will require a pet to be shaved, at the GROOMER'S discretion. Some matting can be so tight, the groom may be refused and referred out to a VET for grooming. HUMANITY BEFORE VANITY.
7: I ALLOW Cute Pawtootie Dog Grooming LLC to take photos of my pet for client files, portfolios, and social media.
8: Prices will always depend on the size of the pet, the type of coat, the condition of the coat, how frequently they get professionally groomed, and also the dog's behavior for grooming. You may see a price increase between grooms depending on any of these factors. Sometimes behavior can not be determined until the groom is complete.
I have reviewed this RELEASE FORM for accuracy and understand the contents of this contract. I have discussed all questions and concerns with the GROOMER. I affirm that I am the rightful legal owner of the pet(s) for which services are being rendered. I authorize this signed contract to be valid approval for future grooming services, permitting the GROOMER to continue service without additional signed contracts or written permission. This agreement is valid for all pets that I currently own or will own in the future.
