General Grooming Waiver
By signing this waiver and leaving your pet(s) in the care of Couture Cuts Pet Salon you understand that you are entering into a contract for the provision of grooming services. You understand and agree that you will be responsible for all costs associated with grooming and you will pay your total promptly upon pick-up. There is a $10.00 fee in addition to the groom cost if we find fleas on your dog.
By signing this waiver you also understand that Couture Cuts Pet Salon (including but not limited to its staff, contractors, owners, etc.) will not be held responsible for any injury to your pet(s), and will not be responsible for any costs associated with any injury to your pet(s). This agreement applies to any and all services provided (past, present, and future) and this waiver will remain in effect until revoked in writing by either party.
Grooming can be a dangerous activity. We are taking sharp instruments near your pet(s), and although we have an impeccable safety record accidents do happen. We will assess the risk involved with the grooming process at the time of drop off and discuss this with you. Couture Cuts Pet Salon employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects of the grooming process, including but not limited to: razor burn, exposure of the quick, reactions to flea treatments and de-shedding dip treatments, hair splinters, and sharp nails (after clipping). By signing this agreement, you (the client) agree to relieve Couture Cuts Pet Salon, and it's agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by Couture Cuts Pet Salon. The pursuit of any veterinary, behavioral, or rehabilitational services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve Couture Cuts Pet Salon and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by Couture Cuts Pet Salon and it's agents.
Lastly, you understand that your pet is in our care, and by signing below accept and acknowledge the responsibility of picking your pet up when you are notified they are ready. Unless a pick-up time is pre-arranged, we will notify you by telephone when your pet is ready. If your pet is not picked up within one hour of this, there will be an additional $5.00 charge per hour added to your grooming total for daycare.
Photo Release Policy
Couture Cuts Pet Salon is proud of the work we do, and we love to show it off! We like to take
before and after photos, and photos of your pet(s) during the groom process for the purpose
of advertising and adding to our portfolio.
We would love to be able to include your pet(s) in our portfolio, on our social media sites and
website, and even in our advertising portfolio! By initialing below you agree to allow Couture
Cuts Pet Salon (including but not limited to its groomers, staff, contractors, and executive) to
use digital and print images of your pet(s) and understand that Couture Cuts Pet Salon will
hold the copyright on these images.
Matted Dog Waiver
Matted coats can cause a variety of skin & health problems. Matted fur does not allow for air circulation to the skin, causing hot spots, bacterial and fungal infections. Fleas, ticks, maggots and other parasites may be lurking in the coat causing further skin infections. Matted fur also pulls and binds, causing pain to your pet when they move or lay on mats. The skin underneath is usually raw and inflamed. Matted coats will not dry properly and can lead to rotting fur and skin.
Due to matting, your dog’s hair may need to be shaved extremely short or to the skin and may show signs of skin irritations and sores due to matting, wet undercoat and dirty coat. In order to take care of a dog that has not been groomed often enough, and is matted, the required grooming sessions may be long, stressful, or painful (please note, more than one grooming session may be required if your dog is severely matted).
Because we believe that de-matting pets is often too stressful for the animal and too dangerous and harmful to the skin, we will not de-matt your pet. We will provide matting relief by shaving the matting out, and, if you desire (and we are able) we will spot shave matting in an attempt to save the coat. However, if the groomer determines that this option is not available you are authorizing Couture Cuts Pet Salon to perform services required to remove the coat in its entirety.
There is a strong chance that your pet’s skin will become irritated during the clipping process. The matted hair rests tightly against the skin – the only way of removing mats is to use a short blade to clip between the skin and mats. Your dog may be nicked and cut because we have to work so closely to the skin to remove the matted coat. Our groomers and staff are very careful, but the possibility of injury exists. Understanding these risks, you are authorizing Couture Cuts Pet Salon to proceed with the groom.
I hereby release Couture Cuts Pet Salon from any liability associated with the above-mentioned process and any and all medical problems that may be uncovered and/or occur during the stripping and de-matting process.
Should my pet need veterinarian care after or during the process, I agree to pay any and all veterinarian fees.
