New Client Application - Brook Working Dogs

In accordance with GDPR we would like to identify what data we will hold and its use. Brook Working Dogs Ltd will retain the following data; your name; telephone number; address; email and your dog’s name; breed; age and medical information. This will be used by Brook Working Dogs Ltd for the purpose of contacting you in relation to your dog training needs/appointments/events that may interest you and will be held for 2 years following inactivity with Brook Working Dogs Ltd. Any paper copies of such details will be incinerated. This information will only be passed to third parties where agreed with you, for example with Vets; dog walkers or other dog related businesses. If you wish to request the removal of your data then please email brookworkingdogs@gmail.com, alternatively you can remove your data by the following methods:

• Method 1) from our service by logging into your client dashboard, and visiting Account > Manage Account > Delete Account
• Method 2) you may request full account deletion through our automated public channel

You will need to enter a valid email address and have access to the email account on file to use method 2. It is your responsibility to ensure the email address on file is valid and active. Once deleted, this information will be removed from active access, and destroyed from backups within 7 days from the time of the deletion.

If you do not have / cannot access to your client account through the previously listed channels, your pet Service Provider will be able to remove your information from their business dashboard directly. Keep in mind that claimed client accounts can only be deleted through the client dashboard or automated removal request form.
Accounts are claimed after one of the following occurs:

• logging in to your client dashboard
• connecting to more than one Service Provider.

Your information is secured behind a password-protected login, which is only accessible to approved employees of Brook Working Dogs Ltd who have been trained in our data handling practices.
Your data is stored using industry standard encryption in transit and at rest, in accordance with the Pawfinity data policy. The storage and processing of data is governed by the Pawfinity privacy policy, found here ( https://www.pawfinity.com/privacy-policy/ ).

Brook Working Dogs employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects of the grooming process, including but not limited to: razor burn, exposure of the quick, reactions to flea treatments and de-shedding dip treatments, hair splinters, and sharp nails (after clipping). By signing this agreement, you (the client) agree to relieve Brook Working Dogs, and it's agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by Brook Working Dogs. The pursuit of any veterinary, behavioral, or rehabilitational services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve Brook Working Dogs and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by Brook Working Dogs and it's agents.
