Dog Daycare and In-Home Boarding Agreement and Waiver Form
I understand that while my dog is fully vaccinated that vaccines are not 100% fool proof and there is still a risk (albeit minimal) that my dog may contract a contagious virus/disease. I agree that should this occur I am responsible for my own pets' care and medical attention. *
I represent that my dog is sociable and has not harmed or shown threatening behaviours towards any person or any other dog. I understand that should my dog display any unwanted behaviors while in the care of Amy's Animal House that staff will remove him/her from the play area. *
I understand that although all dogs are fully supervised that incidents of injuries may occur. This includes that my dog could receive bites, scrapes and scratches from his/her playmates. *
I allow Amy's Animal House staff to contact my/a veterinarian as deemed necessary should any injuries require medical attention. I agree that I am responsible for any medical bills acquired for my own pet. *
I agree to disclose any previous or current medical issues/concerns of my dog so that Amy's Animal House staff can make a determination of suitability for daycare/boarding. *
I release Amy's Animal House from any liability should my dog injure another dog or person and accept medical and legal responsibility of my pets' actions. *
General Liability Waiver
Amy's Animal House employs extensive safety and sanitary precautions to ensure your pets safety and health. Despite reasonable and prudent efforts, some pets may experience common side effects of the grooming process, including but not limited to: clipper burn, exposure of the quick, reactions to flea treatments and de-shedding dip treatments, hair splinters, and sharp nails (after clipping). By signing this agreement, you (the client) agree to relieve Amy's Animal House, and it's agents from any and all liabilities, and/or costs associated with any veterinary care, symptoms or discomfort of your pet related to or presenting after services rendered by Amy's Animal House. The pursuit of any veterinary, behavioral, or rehabilitational services, and the payment there of, is the sole responsibility of the pet owner. Additionally, you agree to relieve Amy's Animal House and its agents from any and all liability associated with damages to person, pet, or property by or relating to services rendered or service equipment used by Amy's Animal House and it's agents.
This waiver for Grooming Services only It does not apply to Boarding or Doggy Daycare
Amy’s Animal House REQUIRES a current Rabies vaccine for all dogs receiving grooming services. For your pet’s own safety, I HIGHLY recommend the Distemper Combination and Bordetella Vaccine. I will accept dogs for grooming services without the Distemper Combination and Bordetella Vaccine; however this is at the owner’s risk. If you would like Amy’sAnimal House to accept your dog for grooming services, without proof of the Distemper Combination and Bordetella Vaccine, please review the following vaccine information and sign below.
Core Vaccines (Recommended for all Dogs) Rabies Vaccine Since rabies is an incurable disease that can even inflict humans, administering a rabies vaccine to your dog is required by law. A rabies vaccine is given when the dog is between 16-18 weeks of age. An additional injection is given after one year, after which boosters are given every one to three years. Distemper Combination Vaccine This vaccine covers diseases like Distemper Virus, Parvovirus, Adenovirus and Parainfluenza Virus in a single injection. This vaccination should be administered to the puppy after 6-8 weeks of age. A booster should also be given every 3-4 weeks, till the puppies are 16 weeks old. An additional injection is given after one year, after which boosters are given every one to three years. Non-Core Vaccines (Depend on Risk Factors) Bordetella Vaccine (Kennel Cough Vaccine) This vaccine fights against the bacteria called Bordetella Bronchiseptica that causes kennel cough in dogs. It should be generally administered to puppies that come in large contact with other dogs (during grooming, boarding and/or training classes). It can be taken either annually or semi-annually, depending on the requirement and the level of risk the dog is open to.
I understand the vaccines recommended by Amy’s Animal House and acknowledge that I accept full responsibility of any condition my dog may contact at Amy’s Animal House without these vaccines. I further agree to not hold Amy’s Animal House liable in any way for any illness related to the lack of these vaccines.
